29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 7022 N LOCUST Gladstone, Missouri.  An employee was exposed to a fall hazard while working at the edge of a low pitched commercial roof  approximately 12-26 feet without guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a commercial worksite located at 7802 Ruskin Way, Kansas City, Missouri. An employee was exposed to a fall hazard by using a ladder which extended approximately 2 1/2 feet above the landing surface of the roof to which he gained access.
