29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: 12033 Maryland Manor Drive, Maryland Heights, MO 63043 Three workers were observed performing roofing activities on a steep 12:12 slope roof without the use of any fall protection system. The workers were exposed to fall hazard of approximately 8 feet from the garage roof to the ground below and approximately 17 feet from the rear main portion of the roof to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Location: 12033 Maryland Manor Drive, Maryland Heights, MO 63043 The employer's training program did not enable each employee to recognize the hazards of falling and was insufficient to train each employee in procedures to be followed in order to minimize fall hazards while employees worked on a 12:12 pitch roof which was 8-17 feet above the next lower level.
