29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Jobsite: 6512 Zupancic Drive, Pittsburgh, PA 15236: On or before August 21, 2013, an employee was working unprotected and exposed to fall hazards of approximately 15 feet 5 inches while on a 4/12 pitched roof. The employee was carrying OSB board.     (b) Jobsite: 6515 Zupancic Drive, Pittsburgh, PA 15236: On or before August 21, 2013, an employee was working unprotected and exposed to fall hazards of approximately 16 feet 1 inches while on a 4/12 pitched roof. The employee was carrying and installing OSB board.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      (a) Jobsites: 6512 and 6515 Zupancic Drive, Pittsburgh, PA 15236: On or before August 21, 2013, the employer did not train their employees on the hazards associated with falling while removing and installing roofing materials on a low-sloped roof and what fall protection methods shall be used for the entire job duration.
