29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed:     On or about August 21, 2013, and at time prior thereto,  the employer did not ensure monthly inspections of hoist chains were inspected for the following overhead cranes:     a)  Polaris 3 ton crane, located on the west end of the shop,   b)  Polaris 3 ton crane, located on the east end of the shop.     Employees were exposed to the hazard of being crushed by a falling load.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about August 21, 2013, and at times prior thereto, the employer did not ensure machine guarding was provided for the following equipment:   a) Pacific #3237, 200 Ton Press Brake had unguarded point of operation and no guards on the sides,   b) Upton UBI Ironworker had an unguarded punch.     Employees were exposed to the hazard of amputations.
