29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:   On or about September 18, 2014 the employer did not ensure powered industrial trucks were examined prior to being placed into service.  Employees were exposed to the hazard of beings struck by the powered industrial truck and/or being crushed by a falling load.   Louvers and Equipment Manufacturer, Inc., dba LEMI was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 933844, citation number 1, item number 2 and was affirmed as a final order on December 23, 2013, with respect to a workplace located at 7007 N. 115th Ave., Owasso, OK 74055.

29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed:   On or about September 18, 2014, and at times prior thereto, the employer did not ensure monthly inspection of hoist chains were inspected for the following overhead cranes:  a) Polaris 3 ton crane, located on the west end of the shop,   b) Polaris 3 ton crane, located on the east end of the shop.   Employees are exposed to the hazard of being crushed or struck-by by a falling load.    Louvers and Equipment Manufacturing, Inc., dba LEMI was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.179(j)(2)(iv), which was contained in OSHA inspection number 933844, citation number 1, item number 3 and was affirmed as a final order on December 23, 2013, with respect to a workplace located at  7007 N 115th East Ave., Owasso, OK 74055.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about September 18, 2014, and at times prior thereto, the employer did not ensure machine guarding was provided for the following equipment:   a) Pacific 3237, 200 Ton Press Brake had unguarded point of operation and no guards on the sides.   Employees are exposed to the hazard of amputations.   Louvers and Equipment Manufacturing, Inc., dba LEMI was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 933844, citation number 1, item number 4 and was affirmed as a final order on December 23, 2013, with respect to a workplace located at 7007 N 115th East Ave., Owasso, OK 74055.
