The employer was exposed to the hazards of struck-by, caught in between, and fall from heights.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:         At the jobsite, on or about August 21, 2013, the employer allowed an employee to operate a rough terrain type forklift without providing the required training, evaluation and certification for powered industrial trucks.  Employees were exposed to the hazards of struck-by and fall from heights.
