29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) On or about July 24, 2013 and at times prior, Massa Auto Pawn as the creating and correcting employer did not ensure that the employees of Mazda of Lakewood in the lower garage area were protected from entrapment in the event of an emergency. Three exit doors in the parking garage were locked and one was barricaded with 2" X 6" boards.

29 CFR 1910.37(b)(1): 29 CFR 1910.37(b)(1): Each exit route was not adequately lighted so that employees with normal vision could see along the exit route: a) On or about July 24, 2013 and at times prior, Massa Auto Pawn as the creating and correcting employer did not ensure that the employees of Mazda of Lakewood in the lower garage area were protected from entrapment in the event of an emergency. Three exit routes were not provided with lighting and illumination.

29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 foot-lamberts (0.21 cd/m2) are permitted. a) On or about July 24, 2013 and at times prior, Massa Auto Pawn as the creating and correcting employer did not ensure that the employees of Mazda of Lakewood were protected from entrapment in the event of an emergency. Three exits signs in the lower garage area were not illuminated with at least 5 foot-candles.
