29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative controls were not utilized: Building 1 - The router operator was exposed to continuous noise at 134.2 % of the permissible daily exposure limit (8-hour time weighted average sound level of 90 dBA) or an equivalent sound level of approximately 92.1 dBA during the 476 minute monitoring period, on or about September 25, 2013. In addition to the mandatory use of effective hearing protection by all exposed employees, an effective hearing conservation program must be maintained as per Citation 1 Item 1b.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: Building 1 - The router operator was exposed to continuous noise at 164.7% of the permissible daily exposure limit (8-hour time weighted average sound level of 90 dBA) or an equivalent sound level of approximately 93.6 dBA during the 476 minute monitoring period, on or about September 25, 2013. Building 1 - The rotomold operator was exposed to continuous noise at 137.4 % of the permissible daily exposure limit (8-hour time weighted average sound level of 90 dBA) or an equivalent sound level of approximately 92.3 dBA during the 478 minute monitoring period, on or about September 25, 2013.
