29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about 03/31/2021, at the fab shop, a bench grinder was in use with its tongue guard adjusted to 3/4 inch from the wheel periphery.   b) On or about 03/31/2021, at the fab shop, a bench grinder was in use with its tongue guard missing and a gap present of 1 and 1/2 inches from the wheel periphery to the peripheral guard.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about 03/31/2021, for Roto Molding Machine # 283, A 480VAC panel box had a broken lock. The electrical box was accessed while live and energized to gain access to breakers/on-off switches for variable frequency drive motors.   b) On or about 03/31/2021, for the Roto Molding Machine # 283, the lockout disconnect for a 480VAC panel box was broken, such that, a lock could not be applied to the disconnect switch.
