29 CFR 1926.503(a)(2):  The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  On or about August 30, 2013, at the worksite located at 4441 East 46th Street in North Little Rock, Arkansas, Fountaine Bleau Apartments, for the benefit of employees working on Building #8, the employer did not ensure employees of subcontractor Gonzalo Diaz were able to recognize, avoid and minimize potentially hazardous conditions associated with falls.  Employees were allowed to work on a pitched roof and dormers with falls of greater than 6 feet without the use of personal protective equipment or proper guarding methods in place to protect against falls.
