29 CFR 1926.1053(a)(19): Where the total length of a climb on a fixed ladder equaled or exceeded 24 feet (7.3 m), the fixed ladder was not equipped with either: (i) a ladder safety device; (ii) self-retracting lifelines with rest platforms at intervals of less than 150 feet (45.7 m); or (iii) a cage or well with multiple ladder sections with ladder section less than 50 feet (15.2 m) in length: (a) On or about 09/01/13 - at 176 Brooks Lane, Irondale, AL, employees were exposed to a fall hazard while climbing and working from a 60-foot fixed ladder that was not equipped with a ladder safety device, a self-retracting lifeline, a cage or well.
