29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. a) Handrailing system was not in place on the outward right hand side ascending from the office, exposing employees to fall hazards.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments. a) On or about August 14, 2013, an employee was trouble shooting inside of an 480 volt electrical disconnect panel using a test meter, the employee was not trained in electrical safety related work practices including but not limited to 29 CFR 1910.331 through 1910.335.

29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs. a) On or about August 14, 2013 the 480 volt electrical disconnect panel was not deenergized during the testing of wires exposing employees to live parts and/or electrical shocks.
