29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      a)  SGM Construction LLC at 47th Ave. and N. Willow St. Building P, Denver, CO 80230:  On or before August 31, 2013, the employer did not ensure that an employee wore safety glasses using a pneumatic nail gun for siding.  Condition exposed this employee to the hazard of projectiles to the eyes.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift:  (Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system, and that harnesses are required.)      a)  SGM Construction LLC at 47th Ave. and N. Willow St. Building P, Denver, CO 80230:  On or before August 31, 2013, the employer did not ensure that an employee was tied off while working from personnel platform attached to a Genie rough terrain forklift, model S-65, while siding this building.  Condition exposed this employee to a fall hazard of approximately 25 feet.
