29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     On or about August 27, 2013, at the above mentioned jobsite, an employee was exposed to a fall hazard of approximately 10 foot with a ladder not extended 3 feet above the landing and not secured.     PLEASE NOTE: AN ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about August 27, 2013, at the above mentioned jobsite, an employee was exposed to a 10 foot fall hazard conducting roofing repair work on a townhouse without a means of fall protection.    The Praxis Industries, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection Number 316325968, Citation Number 1, Item Number 2 and was affirmed as a final order on 05/02/2013 , with respect to a workplace located at 7500 NW 4th Place, Margate, FL 33063 .    PLEASE NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT ARE REQUIRED FOR THIS VIOLATION.
