29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) On or about August 29, 2013, employees of Potosino Construction were exposed to fall hazards in excess of 6 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On or about August 29, 2013, the employer did not ensure that eye and face was worn by employees when using machines that presented potential eye or face injury. An employee was driving nails onto roof plywood sheets using a pneumatic nail gun without safety glasses or any other eye or face protection.
