29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       (a)  Jobsite, Marquis Place, Export, PA:  On or about 09/05/13 an employee engaged in residential construction activities, approximately 30 feet above the ground, was not protected from falling by the use of guardrails, safety nets or personal fall arrest system.

29 CFR 1926.503(c)(3): Retraining was not conducted when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.   Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part:     (a) Jobsite, Marquis Place, Export, PA:  On or about 09/05/13 retraining had not been conducted when an employee was observed working at heights up to approximately 30 feet without fall protection indicating the employee, who had been previously trained, had not retained the requisite understanding or knowledge to recognize fall hazards and use fall protection systems.
