29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    A)2 Normandy Court, Warren, NJ 07059: Employee was exposed to fall hazard of approximately 22 feet from the working level to the ground due to missing guardrails and endrails and/ or netting on a pump jack scaffold.    Condition observed on or about September 10, 2013.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:     a) 2 Normandy Court, Warren, NJ 07059: Employee was observed performing siding operations at a height of approximately 24' to the ground below with no fall protection in place, exposing them to fall hazards.    Condition observed on or about September 10, 2013.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       a) 2 Normandy Court, Warren, NJ 07059: The ladder provided was not secured and did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof.         Condition observed on or about September 10, 2013.
