Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: a) Shape molding building, and maintenance shop: On September 11, 2013 and at times prior thereto the industrial storage rack system in use was not inspected and maintained. Safety bolts or other safety devices on the beams were either not in place, damaged, or were not positively engaged with the supporting upright columns to prevent dislodging of the beams. 29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: a) Shape molding building: On September 11, 2013 and at times prior thereto walking and working areas were not kept in a neat and orderly condition. Regularly scheduled housekeeping activities were not performed in order to keep fugitive product, personal garbage, and generally discarded equipment, and materials from accumulating around the machines and walkways. b) EZ Router: On September 11, 2013 and at times prior thereto regularly scheduled housekeeping activities were not performed to keep excessive dust and shavings produced by the process from accumulating on electrical appliances, the work station, ledges, and surrounding floor space. FMI-EPS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 596720, citation number 2, item number 2, and was affirmed as a final order on February 6, 2013, with respect to a workplace located at 280 Rose Street, Jerome, ID 83338.
