Where wet processes are used, drainage was not maintained.  a)    At EPS molding/press machine #5, On Feb. 25, 2015, and at times prior thereto, the floor where the product is discharged was wet and had no provision for draining the water away .  b)    At EPS molding/press machine #5, On Feb. 25, 2015, and at times prior thereto, the floor where the hydraulic pump system was wet and had no provision for draining the oil and water from water and leaking hydraulic oil causing the floor to be very slippery.
