29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the gray New England Butt Co. wire braider machine, located closest to the braider room door, were exposed to rotating part hazards.  The employer did not assure a rotating shaft and braiding spools were guarded.      b) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 10, 2013, employees operating the green New England Butt Co. wire braider machine, located second from the braider room door, were exposed to rotating part hazards.  The employer did not assure a rotating shaft and braiding spools were guarded.    c) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the gray New England Butt Co. wire braider machine, located third from the braider room door, were exposed to rotating part hazards.  The employer did not assure a rotating shaft and braiding spools were guarded.    d) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the Kingsley machine (model MP2000ET; s/n KM156) were exposed to injuries associated with rotating parts.  The employer did not assure that the rotating shafts, used to spool imprinted wire, were guarded to protect employees.       e) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the Kingsley machine, located on the left facing side, were exposed to injuries associated with rotating parts.  The employer did not assure that the rotating shafts, used to spool imprinted wire, were guarded to protect employees.    f) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the Tulsa imprint machine (model MMC-P302; s/n 2504) were exposed to injuries associated with rotating parts.  The employer did not assure that the rotating shaft, used to spool imprinted wire, was guarded to protect employees.    g) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the ink-jet wire printing machine were exposed to injuries associated with rotating parts.  The employer did not assure that the rotating shaft, used to spool printed wire, was guarded to protect employees.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 6, 2013, employees operating the Kenco wire terminal crimping machine (model 3K-7-150; s/n 85-3823) were exposed to amputation injuries in that the point of operation was not adequately guarded.    b) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 5, 2013, employees operating the #1 Packard/Kenco wire terminal crimping machine were exposed to amputation injuries in that the point of operation was not adequately guarded.    c) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 1, 2013, employees operating the #2 Kenco wire terminal crimping machine (model 3K-7-150; s/n 92-4898) were exposed to amputation injuries in that the point of operation was not adequately guarded.    d) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 5, 2013, employees operating the #5 Amp Inc. wire terminal crimping machine (s/n 63827) were exposed to amputation injuries in that the point of operation was not adequately guarded.    e) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 5, 2013, employees operating the #6 Amp Inc. wire terminal crimping machine (s/n 126108) were exposed to amputation injuries in that the point of operation was not adequately guarded.    f) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 1, 2013, employees operating the #7 Amp Inc. wire terminal crimping machine (s/n 129213) were exposed to amputation injuries in that the point of operation was not adequately guarded.    g) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about November 1, 2013, employees operating the #8 Amp Inc. wire terminal crimping machine (s/n 125910) were exposed to amputation injuries in that the point of operation was not adequately guarded.
