Employees that accessed the hazard area of the machine for set-up were not trained to control hazardous energy sources.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the gray New England Butt Co. wire braider machine, located closest to the braider room door, were exposed to rotating part hazards. The employer did not assure a rotating shaft and braiding spools were guarded. b) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 10, 2013, employees operating the green New England Butt Co. wire braider machine, located second from the braider room door, were exposed to rotating part hazards. The employer did not assure a rotating shaft and braiding spools were guarded. c) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the gray New England Butt Co. wire braider machine, located third from the braider room door, were exposed to rotating part hazards. The employer did not assure a rotating shaft and braiding spools were guarded. d) EWH Spectrum, LLC - Bellefontaine, Ohio: On or about September 16, 2013, employees operating the Kingsley machine (model MP2000ET; s/n KM156) were exposed to injuries associated with rotating parts.
