29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  On or about September 23, 2013 and times prior thereto, an employee of Young Well Service, Inc. performed pipe latching from the derrick platform on Workover rig for salt water injection well David B. Kuhn No. 3 located at or near Section 6 T155 R 27W 27 in Texarkana, Arkansas.  The employer had selected and had employees use a 1" flexible steel tail rope with the use of a body belt which did not protect the employees in the event a fall had occurred.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  On or about September 23, 2013 and times prior thereto, employees of Young Well Service, Inc. performed workover services with the use of workover rig#11 for the David B. Kuhn No. 3 salt water injection well located at or near Section 6 T155 R 27W 27 in Texarkana, Arkansas.  The employer had not provided employees exposed to fall hazards and using fall protection in the derrick platform with training, exposing employees to fall hazards of 50 feet to the ground level below.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:  On or about September 23, 2013 and times prior thereto, employees of Young Well Service, Inc. performed workover services on the David B. Kuhn No. 3 salt water injection well with the use of a workover rig at or near Section 6 T155 R 27W 27 in Texarkana, Arkansas.  The employees performed service operations which were not in near proximity to an infirmary, clinic, or hospital and the employer did not ensure there was a person onsite trained to render first aid.
