29 CFR 1910.22(a)(3):  Every floor, working place, and passageway was not kept free from protruding nails, splinters, holes, or loose boards.    a) Laundry Room- Employees working around the laundry machines were exposed to an open drain pit partially covered by a loose board, exposing employees to a tripping hazard, on or about 09/24/13.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.23(a)(5): Every pit floor opening was not guarded by a floor opening cover of standard strength and construction.     a) Laundry Room- Employees working around the laundry machines were exposed to an open drain pit partially covered by a loose board, exposing employees to caught-in hazard, on or about 09/24/13.
