29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   a. for the employee working in the silk screen printing area located at 4611 4th Street N. in St. Petersburg (FL), as noted on or about 09/26/2013.  The employee used a a half face, respirator, with dual organic vapor cartridges, for protection against the health hazards associated with chemicals in the silk screen area.  No written program had been established and the employee had not been medically qualified, fit-tested or trained for the use of the respirator.
