29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998] a. Dock Area - On or about October 18, 2013, the employer did not provide employees with suitable facilities for quick drenching or flushing of the eyes to employees who were potentially exposed to splashing hazards of sulfuric acid (H2SO4). Employees were potentially exposed to eye irritations.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a. Warehouse and Dock Areas - On or about October 18, 2013, the employer did not ensure that employees required to operate (Toyota with Serial # 11743 and model # 7FBEU20) electric powered industrial trucks received training and evaluation on the safe operation of industrial trucks.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a. Warehouse and Dock Areas - On or about October 18, 2013, the employer did not train and evaluate each operator required to operate electric powered industrial trucks (Toyota with Serial # 11743 and Model # 7FBEU20) with a certification of training that included the name of the operator, the date of the training, the date of the evaluation, and the identity of the person performing the training.
