29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    Receiving area - On or about October 23, 2013, employee(s) were required to operate powered industrial trucks. The employer had not certified that an employee evaluation of each powered industrial operator's performance had been conducted at least once every three years. Employees were thereby exposed to the hazards associated with powered industrial trucks.
