Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level, was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Lake Construction LLC at 2002 West 76th Ave. Building # 26, Denver, CO 80221: On or before October 23, 2013, the employer did not ensure that employees were protected from falling when using the pump jack scaffold in the following instances: 1) The guardrail system did not include midrails; 2) The guardrail system did not include endrails; 3) The 2 x 4 top rails would not support a 200 pound load as specified in 29 CFR 1926.451(g)(4) because: a) the guardrails were supported only by the vertical pump jack stands that were placed up to 15 feet apart, and the distance between vertical supports exceeded 8 feet as specified in 1926 Subpart M Appendix B; b) the top rails were not properly secured to the uprights; Employees were not wearing personal fall arrest systems. Condition exposed employees to fall hazards of up to 18 feet.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters (GFCIs) for personal protection: a) Lake Construction LLC at 2002 West 76th Ave. Building # 26, Denver, CO 80221: On or before October 23, 2013, the employer did not ensure that a GFCI was in use when employees cut siding using a 120 volt Dewalt circular saw model DWS535, serial number 153739, that was not double insulated. Condition exposed employees to an electrical shock hazard.
