29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation of the correction of the hazards created by the fact that the Catepillar TH63 Telehandler was not constructed in accordance with the design of a qualified person:  a. The platform floor did not have a slip-resistant surface;  b. The floor dimensions of the platform had a width greater than the overall width of the forklift;  c. The maximum working load, including personnel and equipment, was not posted on the platform;  d. There was no means to securely attach the platform to the lifting mechanism, and to prevent the platform from inadvertent pivoting.
