29 CFR 1926.20(b)(2): The employer failed to initiate and maintain programs that provided for frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employer: At the front exterior of the printing store on or about November 4, 2013, and at times prior thereto, an inspection was not completed or the person that completed the inspection was not a competent person. The owner of the company did not know OSHA's regulations for ladders or fall protection on low sloped roof work.

29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system.]: At the front exterior of the printing store on or about November 4, 2013, and at times prior thereto, there was no fall protection for employees working on top of a low sloped canopy. It was 11 feet from the canopy to the asphalt below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: At the front exterior of the printing store on or about November 4, 2013, and at times prior thereto, a 20 foot extension ladder did not extend at least three feet above the canopy. The ladder extended about six inches above the canopy. Employees used the ladder to access the top of the canopy that was 11 feet above the asphalt below.
