29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:     a) On or about November 1, 2013, Amaya's Roofing LLC did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. An employee was observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge.        Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:       a) On or about November 1, 2013, employees of Amaya's Roofing LLC were exposed to fall hazards in excess of 6 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place.            Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").      Amaya's Roofing LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 515738, citation number 1, item number 2 and was affirmed as a final order on August 7,2012, with respect to a workplace located at 4230 Carnoustie Ct. Colorado Springs, CO 80909.
