29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a)  On or about September 29, 2014, employees were exposed to falls of approximately 9 feet, while accessing a roof from a portable ladder, that did not extend at least three feet above the working surface, while conducting reroofing activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about September 29, 2014, employees were exposed to falls of approximately 9 feet, while conducting reroofing activities, on a 4/12 pitch roof, without the benefit of a fall protection system being in place.       Amaya's Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 990970, Citation 1, Item 1, and was affirmed as a final order on September 9, 2014, with respect to a workplace located at 7461 Isabell Circle, Arvada, CO 80007.      Amaya's Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 946396, Citation 2, Item 1, and was affirmed as a final order on December 06, 2013, with respect to a workplace located at 3213 N. Arcadia Street, Colorado Springs, CO.
