29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) Residence: On or about October 23, 2013, and at times prior, Premier Roofing, LLC., as controlling employer, did not ensure that Big Valle Roofing, LLC., protected their employees from fall hazards ranging in height from 8.9 feet to over 19.2 feet while installing roofing material.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).    Premier Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 106493, citation number 01, item number 003, and was affirmed as a final order on April 4, 2012, with respect to a workplace located at 11365 Cranston Ave, Peyton, CO 80831.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder length did not allow the side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a)  Residence: On or about October 23, 2013, and at times prior, Premier Roofing, LLC., as a controlling employer, did not ensure that Big Valle Roofing, LLC., protected their workers from falling 10 feet while using a ladder that did not extend above the first-story roof edge and was not secured at the upper landing level.      b) Residence: On or about October 23, 2013, and at times prior, Premier Roofing, LLC., as a controlling employer, did not ensure that Big Valle Roofing, LLC., protected their workers from falling 19.2 feet while using a ladder that did not extend above the second-story roof edge and was not secured at the upper landing level. 29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Residence: On or about October 23, 2013, and at times prior, Premier Roofing, LLC, as controlling employer, exposed two employees of Big Valle Roofing, LLC., to fall hazards when the frequent inspection conducted in the afternoon did not identify and correct hazards related to improper placement of ladders and workers exposed to falls greater than 6 feet without fall protection.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
