29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  At the project site on November 5, 2013, an employee used a Little Giant type ladder in the stepladder position, and stood on the second rung from the top.  The Danger label on the ladder states WHEN USED AS A STEPLADDER DO NOT STAND ON OR ABOVE THIS RUNG. YOU CAN LOSE YOUR BALANCE.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  At the project site on November 5, 2013, the employer did not provide a training program for each employee using ladders and stairways, as necessary. An employee used a Little Giant type ladder in the stepladder position, and stood on the second rung from the top.  The Danger label on the ladder states WHEN USED AS A STEPLADDER DO NOT STANDON OR ABOVE THIS RUNG. YOU CAN LOSE YOUR BALANCE.
