At the project site on November 6, 2013, an employee did not wear a full body harness with a lanyard attached to the boom or basket while working from the basket of a National Crane Boom, mounted on a Ford L8000 Tuck, License 3874FSO.

29 CFR 1926.502(d)(6)(v): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.    At the project site on November 6, 2013, employees working from the basket of a National Crane Boom mounted on a Ford L8000 Tuck, License 3874FSO, attached their snaphooks onto a L shaped metal round stock welded to the boom, thus creating a potential for unintentional disengagement in the event of ejections from the lift platform.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    At the project site on November 6, 2013, employees exposed to fall hazards were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Three employees observed working from the inside of the National Crane lift basket, mounted on a Ford L8000 Tuck, License 3874FSO.  Two employees did not properly attach lanyards to a suitable anchorage device, and one employee was not using a personal fall arrest system, thus exposing employees to a fall of about 10 feet to the asphalt.
