29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):       a)  Jobsite, 635 East Pittsburgh Street, Greensburg, PA 15601:  Observed on or about November 05, 2013, the working level of a three level mobile scaffold was not fully planked.  Employees using the scaffold to access ladder jack scaffold platforms were exposed to a fall hazard of approximately 17 feet to the paved surface below.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:   a)  Jobsite, 635 East Pittsburgh Street, Greensburg, PA 15601:  Observed on or about November 05, 2013, a scaffold on the west side of building was not supported by a stable surface.  The right end (as facing building) of a ladder jack platform was resting on the step frame of a mobile scaffold.  Employees working from the scaffold plank and using the scaffold plank to access the roof of the building were exposed to a fall hazard of approximately 17 feet to the paved surface below.

29 CFR 1926.452(w)(8): Where leveling of the scaffold was necessary, screw jacks or equivalent means were not used:    a)  Jobsite, 635 East Pittsburgh Street, Greensburg, PA 15601:  Observed on or about November 05, 2013, a mobile scaffold being used by employees to access the roof was not equipped with screw jacks or equivalent means for leveling the scaffold.  The scaffold was resting on wheels, two of which were resting on pieces of wood.  Employees working from a scaffold plank resting on the mobile scaffold were exposed to a fall hazard of approximately 17 feet to the paved surface below.
