29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not being used. Crossbraces shall not be used as a means of access:     On or about November 14, 2013, employees on the South side of a residential house were exposed to a fall greater than 10 feet while climbing a scaffold frame to gain access to stone frame a chimney on a roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:       On or about November 14, 2013, employees without fall protection on the South side of a residential house were exposed to a fall greater than 6 feet in height to the unpaved ground below, while stone framing a chimney with a roof pitch of 6:12.any of those systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:     On or about November 14, 2013, employees without fall protection on the South side of a residential house were exposed to a fall greater than 6 feet in height to the unpaved ground below, while stone framing a chimney with a roof pitch of 6:12.
