29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  In the rear of the building on or about November 18, 2013, there was no fall protection for a 6 foot opening in the guardrails on the mezzanine.  There was barricade tape across the opening.  It was 11 feet, 2 inches from the mezzanine to the concrete floor below.  Employees were on the mezzanine to test the sprinkler system for leaks and had to walk near the opening to access the work area.
