29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  At Andersen Air Force Base, inside building 18001 on November 13, 2013, and at times prior thereto, the female end ground plug of a 50 foot extension cord was cracked.  The cord was used in conjunction with a Bosch 11255VSR BULLDOG Xtreme, 1-Inch Rotary Hammer.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At Andersen Air Force Base, inside building 18001 on November 13, 2013, and at times prior thereto, the path to ground was not permanent and continuous. A 50 foot extension cord had an "Open Ground." The cord was used in conjunction with a Bosch 11255VSR BULLDOG Xtreme, 1-Inch SDS-plus D-Handle Rotary Hammer.
