29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Throughout the Job Site - As the controlling employer, construction debris such as drywall scraps, wood, cardboard, buckets and other miscellaneous materials was not cleared from the work area and from stairs and aisles.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: First Floor - As the controlling employer, the assurance of installing railings around the stairway opening was not done nor was any other type of fall protection provided.

29 CFR 1926.34(b): Exits and access to exits were not marked by a readily visible sign: Job Site - In the basement there were three exits that had no sign indicating they were exits. Portland Bjuilders was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 112524830, citation number 1, item number 1, and was affirmed as a final order on 3/25/2009, with respect to a workplace located at Canal Street in North Berwick, ME.
