29 CFR �1910.160(b)(2):  If for any reason a fixed extinguishing system becomes inoperable, the employer shall notify employees and take the necessary temporary precautions to assure their safety until the system is restored to operating order:    Although not a specifically required fixed extinguishing system, on or about November 18, 2013, at the ExxonMobil King Ranch Gas Plant located in Kingsville, Texas, the employer did not notify persons who may use the fire hydrants that some of the fire hydrants on the employers premises were not in working order.

29 CFR �1910.119(f)(1):  The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information:    On or about November 18, 2013 in the Refrigerated Absorption De-Methanization Unit (RADU) at the ExxonMobil King Ranch Gas Plant located in Kingsville, Texas, the employer did not ensure that its employees followed its established Critical Safety Device Bypass Guidelines when they bypassed the Low level Shutdown interlock associated with stills 1108A/B.
