Condition observed on or about 11/19/13.  e) The employer failed to provide documentation and certification for abatement of citation 1 item 5 in which an employee carried out a 4 foot x 8 foot sheet of plywood overhead up the 32 foot aluminum extension ladder and then attached it to the stud wall. Condition observed on or about 11/19/13.  f) The employer failed to provide documentation and certification for abatement of citation 1 item 6 in which employees had not been trained or retrained in the requirements to use their ladders in accordance with the OSHA standards. Condition observed on or about 11/19/13.  g) The employer failed to provide abatement and certification for abatement of citation 2 item 1 in which employees without proper fall protection was working on a roof at an edge exposed to an approximate 11 to 30 foot fall to the ground.
