29 CFR 1926.451(d)(12)(iii): Wire rope clips used on suspension scaffold ropes were not inspected and retightened to the manufacturer's recommendations:  On or about 11/21/2013, at the above addressed jobsite, two employees operating a suspension scaffold were removing beams from the exterior of an 8-story hotel, whereas the outrigger tieback clips were not inspected and retightened to the manufactures' torque recommendations after initial loading, and at the start of each work shift, and after the initial and each subsequent assembly.  PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 11/21/13, at the above addressed jobsite, employees working from a suspension scaffold were removing exterior beams of an 8-story hotel whereas they were using a retractable lanyard attached to shock absorbing lanyard exposing them to a free fall hazard of more than 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 11/21/13, at the above addressed jobsite, the employer did not provide adequate training to their employees to recognize fall hazards while operating a suspension scaffold attached to a fall protection system that was rigged to allow an employee to free fall more than six (6) feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  On or about 11/21/13, at the above addressed site, employees were using a stairway with four risers approximately 36" in height to access the roof area, whereas the stairway was not equipped with a stairrail system.
