29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: 1. Receiving Room: The circuit breaker panel next to the assembly table had three knockouts missing. 2. Mop closet: The outlet on the wall where the communication system is located was missing the cover plate.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors: 1. Receiving room: Two coils of non-metallic sheathed cable were run through the drop ceiling and were hanging on the wall. 2. Design Center: An extension cord was plugged into an outlet on a post, run up into the drop ceiling and down behind the desks to provide power to the workstations and other electronics there.
