29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Boiler Room: Employer did not provide and ensure the use of protective shield/barrier or thermal protective clothing to protect employees from thermal burns while performing inspections and maintenance of the boiler such as the blow-down operation. On October 23, 2013, an employee sustained a first and second degree thermal burn on the legs, torso and arms while performing a blow-down of the boiler.

29 CFR 1910.262(c)(9): Pipe(s) carrying steam or hot water for process or servicing textile machinery, within 7 feet of the floor or working platform, were not covered with heat insulating material or otherwise guarded: Boiler Room: Piping for the boiler that carries hot water and steam including, but not limited to, the blow down pipes that were located within 7 feet of the work platform were not insulated or properly guarded to protect employees from thermal burns when they were engaging in operations such as blow-down. On October 23, 2013, an employee sustained a first and second degree thermal burn on the legs, torso and arms while performing a blown-down of the boiler.
