29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Establishment, Production Area - On or about June 24, 2025, an employee was operating a Sheridan press model 1275, making repetitive presses of leather hides when he reached into the machine to unjamm a leather hide while the press was still operating. The employer did not have a LOTO program established to shut down of the press exposing employees to amputation hazards.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles:  (a) Establishment, Production Area - On or about June 24, 2025, an employee was operating a Sheridan press model 1275, making repetitive presses of leather hides, when he reached into the machine to unjam a leather hide while the press was still operating.  The press was not protected by a point of operation guard and the employee was exposed to amputation hazards.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   (a) Establishment, Production Area - On or about June 24, 2025, an employee was operating a Sheridan press model 1275, making repetitive presses of leather hides, when he reached into the machine to unjam a leather hide while the press was still operating, resulting in employee sustained finger tip amputations.  The employer failed to report work a work-related incident resulting in amputation and in-patient hospitalization to OSHA within 24 hours.
