29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Jobsite: The employee, working from a porch roof installing insulation, exposed to an 11 foot fall hazard was not protected from falling off the 4 pitch roof. On or about 11 December 2013 an employee sustained an ankle injury when the porch roof he was working from pulled away from the house and fell to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Jobsite: The employee who was exposed to fall hazards of approximately 11 feet above the ground, while working from a porch roof, were not fully trained in fall hazards and the procedures to be followed to minimize those hazards. Hazards such as, but not limited to, procedures for erecting, maintaining, disassembling, and inspecting fall protection systems; use and operation of personal fall arrest systems.
