29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o).  On or about December 12, 2013 and times prior thereto, the lubrication pump mounted on top of 55 Gallon drum for dispensing machine oil fluids did not have a guard fully enclosing the belt. Exposing employees to moving parts, pinch points  and caught in injuries.

1910.305(g)(1)(iv)(A) Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   On or about December 12, 2013 and times prior thereto, A flexible cord was used to supply power to a metal cutting band saw.  The cord was attached to metal structure across metal beams in the ceiling. Employees were exposed to shock and fire hazards.

1910.305(g)(2)(iii) Flexible cords were not connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws:   On or about December 12, 2013 and times prior thereto, a flexible cord used for the metal flattener machine was found with its outer insulation pulled away from the plug's strain relief. The condition exposes employees to arcing effects and electrical hazards.

On or about December 12, 2013 and times prior thereto portable fire extinguishers were not visually inspected at least monthly: Located throughout the facility both in stationary mounted locations and fire extinguishers placed out for spot use were found not to have updated tag for visual monthly inspections subjecting employees to fire hazards.
