29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded.    Bay area; on or about 12 December 2013 and times prior; a floor opening 8 inches in length by 5 inches in width by 22 inches in depth was not guarded.  This condition exposed employees to a tripping hazard.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    Bay area; on or about 12 December 2013 and times prior; live exposed 120 volt electrical wires were not guarded in stalls labeled 33-34 and 35-36.  This condition exposed employees to an electrical shock and electrical burn hazards.

29 CFR 1910.305(j)(2)(vii): A receptacle installed in a wet location where the product intended to be plugged into it was to be attended while in use (for example, portable tools) did not have an enclosure that was weatherproof when the attachment plug cap was removed.  Wash bay; on or about 12 December 2013 and times prior thereto, the employer did not ensure that the electrical outlet in a wet location was protected. This condition exposed employee to an electrical shock hazard.
