29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      Building 7301, Outdoor Recreation Center:  On or about December 12, 2013 and at times prior thereto, the operator of the Hyster forklift within the facility could not provide documentation that demonstrated the successful completion of powered industrial truck operations training.  This condition exposed employee within the facility to struck-by and caught-between hazards.

29 CFR 1910.304(b)(2)(ii): Receptacles and cord connectors having grounding contacts were not effectively grounded:   Building 7301, Gibson Refrigerator:  On or about December 12, 2013 and at times prior thereto, the grounding contact of the appliance was defeated by using a two-wire extension cord.  This condition exposed employees to electrocution hazards in the event the appliance ground-faulted.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm.     Building 7301, Outdoor Rec Center, Maintenance Bay:  On or about December12, 2013 and at times prior thereto, metal pallet storage racks did not have the maximum safe load limits posted. Pallet storage racks were not secured to the wall or floor.  Several pallet storage racks had damage to the support legs and cross braces. This condition exposed employees to a struck by hazard.
