This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  673 FSS, Bldg. 8515, Warehouse: On or about 13 December 2013 and times prior thereto, a 100 foot, orange extension cord had cut insulation.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm.    673 FSS Bldg. 8515, Warehouse:  On or about 13 December 2013 and times prior thereto seven 3 foot by 9 foot storage racks were not secure to the wall or floor. Pallet racks are located in a seismically active zone. Pallet storage racks did not have the maximum safe load limits posted. This condition exposed employees to a struck by hazard.
