Northeast door; on or about 13 December 2013 and times prior thereto, the employer did not ensure that the exit path was clear. Snow and ice was accumulated on the exit path. This condition exposed employees to delays in exiting the building and smoke inhalation and fire hazards.

Warehouse area; on or about 13 December 2013 and times prior; the doorway to the Airmans attic was not marked "Not an Exit" or identified by signage marking its actual use. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

Foreman office; on or about 13 December 2013 and times prior thereto, the employer did not ensure that workplace was maintain in a clean condition. The office area had evidence of water intrusion; ceiling tiles were damaged and laying on the floor. This condition exposed employees to contact with toxic, caustic or noxious substances.
